Telling them apart is often the difference between treatment that finally works or drops that never quite do.

Evaporative: the tears escape too fast

In evaporative dry eye, tears evaporate too quickly, often because their oily part is unstable due to a problem with the meibomian glands. The amount of tears can be normal and still not be enough.

The cause: an unstable oily part of the tear, often from a problem with the eyelid glands, which isn’t just about them getting “blocked” but also about changes in what they produce, inflammation or changes in their shape.

How it feels: burning, grittiness and watering, worse on screens and by evening.

What helps: restoring the oil layer – piling on more watery drops alone won’t hold.

Some treatments: warm compresses and eyelid cleaning, drops with a little oil and, depending on the case, in-clinic procedures. IPL is used in some people with a gland problem; other techniques, such as Tixel, depend on their evidence and on whether they’re authorised in each country.

Aqueous-deficient: not enough tears

In the water-shortage type, there are few tears because the gland that makes them doesn’t produce enough. It’s less common and is linked more to age, certain medicines or conditions such as Sjögren’s syndrome.

The cause: too little watery tear from the lacrimal gland.

How it feels: constant dryness and soreness, sometimes alongside a dry mouth.

What helps: boosting and preserving tear volume, and treating any underlying condition.

Some treatments: drops and gels to top up volume and to treat any underlying cause. In selected cases, small plugs that hold tears in can be considered, but inflammation and drainage should be checked first.

Many people have both

The two types often mix: tears with an unstable oily part and, at the same time, a low amount of tears. That’s why looking at everything helps more than guessing (TFOS DEWS III recommends taking several findings into account, not just one).

Why the difference matters

The treatments genuinely differ: one targets the oil, the other targets tear volume. That’s why reaching for ever more watery drops rarely fixes the evaporative type, and why getting the diagnosis right tends to bring relief far faster. It saves months of trying the wrong thing.

How Smart Dry Eyes tells them apart

We measure your tear volume, how fast the film evaporates, and the state of your meibomian glands – so we know whether you’re dealing with the evaporative type, the aqueous-deficient type, or a mix of both, and can treat accordingly rather than by trial and error.

Frequently Asked Questions

Which type is more common?

The evaporative type is usually the most common, but the exact proportion depends on who is studied and how it’s defined. Also, many people have a mix of the two.

Can the type change over time?

Yes, it can develop into a mixed form. That’s why periodic check-ups are a good idea.

Why don’t my drops help?

Often because they do not match your type – for example pure water drops for an oil deficiency. The right classification is crucial.

Not sure which type of dry eye you have?

A quick questionnaire can help show whether your dry eye is evaporative, aqueous or mixed and what’s worth checking.

Find out which type of dry eye you might have

Prefer to be seen in person?
