The muscles in the lid squeeze the gland with every blink, and at the temperature the lid sits at the oil is thin enough to flow. In meibomian gland disease both of those change.<\/p><p>Bacterial lipases split the meibum into free fatty acids and the lipid mix shifts towards more saturated species, so the melting point rises and at ordinary lid temperature the secretion is a waxy paste rather than an oil. At the same time the gland opening narrows and keratinises. The result is a plug.<\/p><\/div><div class=\"txlede-b\"><h3>What happens behind the plug<\/h3><p>What follows is the part patients are rarely told. The gland does not stop producing. It keeps secreting behind the blockage, pressure builds inside the duct, and the secretory sacs at the far end begin to atrophy under it.<\/p><p>On meibography, which photographs the glands through the everted lid, this shows as glands that have shortened, thinned, or gone altogether. Expression relieves the back-pressure that drives that process, which is why it is done early rather than as a last resort.<\/p><\/div><\/div><div class=\"txcall\"><p>Heat first, then expression, always. Expression without warming is painful and largely ineffective against a solid plug. Warming without expression lets the oil cool and set again in the same place.<\/p><\/div><\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">03<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"the-instruments\">The instruments<\/h2>\n<p class=\"txdevnote\">Smart Dry Eyes is not tied to any manufacturer. The warming is not preparation for your comfort; it is what makes the oil movable.<\/p><\/li><li><h3>During<\/h3><p>A second dose of anaesthetic, then the plate or paddle is placed behind the lid and a swab presses from outside, working along the lid in short sections. Pressure rather than pain: in the published trial the median score was 2 out of 10. About five minutes.<\/p><\/li><li><h3>After<\/h3><p>The lids are cleaned and an antibiotic drop is often given. Lids can look pink and feel tender for a few hours. You can leave immediately.<\/p><\/li><li><h3>The course<\/h3><p>A typical published protocol is six sessions every two weeks over twelve weeks. Your centre will set the interval from what actually comes out of the glands at each visit.<\/p><\/li><\/ol>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txsplit\"><div><div class=\"txhead\"><span class=\"txnum\">05<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"what-the-evidence-shows-for-gland-expression\">What the evidence shows for gland expression<\/h2><\/div><div><div class=\"txev\"><div class=\"txevr\"><p class=\"k\">Randomised trial, 2026, 62 patients<\/p><div class=\"v\"><p>A randomised trial published in 2026 compared expression plus lid hygiene against lid hygiene alone in 62 patients, with the assessor masked. Expression was better on symptom scores at every time point, better on the quality of the meibum at twelve weeks, and better on tear break-up time by a little over a second at twelve weeks. An inflammatory marker in the tears fell by 47 per cent in the expression group only. No adverse events occurred.<\/p><\/div><\/div><div class=\"txevr\"><p class=\"k\">Where lid hygiene caught up<\/p><div class=\"v\"><p>The result that deserves attention is the one that qualifies it. Gland dropout fell faster in the expression group at four weeks, but by weeks eight and twelve the two groups were statistically indistinguishable on gland structure. Diligent lid hygiene, done properly, caught up. Expression accelerates recovery and does more for how you feel; it is not the only thing that moves the structure.<\/p><\/div><\/div><div class=\"txevr txevr--pos\"><p class=\"k\">What this means for you<\/p><div class=\"v\"><p>It also produced the single most useful finding for patients on this whole site: both groups showed real gland regeneration. Glands that look shortened or missing on meibography are not necessarily gone for good. Anaesthetic drops are used, and expression that is genuinely painful usually means the warming step was inadequate.<\/p><\/div><\/details><details><summary>Can I do this myself?<\/summary><div class=\"a\"><p>No, and pressing hard on your own eyelids can damage the lid margin without emptying anything. The force needed is applied against a plate held behind the lid, which is not something you can do to yourself, and the practitioner is watching what comes out to judge how hard to press.<\/p><\/div><\/details><details><summary>Is it the same as LipiFlow or iLux?<\/summary><div class=\"a\"><p>Those devices combine heating and compression in one step, automatically.
