Staphylococci living there secrete a polysaccharide matrix around themselves, and the result is a biofilm: adherent, structured, and completely unlike loose debris. The closest everyday comparison is dental plaque.<\/p><p>You cannot rinse it away, and mouthwash alone does not remove it; a toothbrush does, because it applies shear. The same is true here, and it is the reason lid wipes disappoint patients who use them diligently for months.<\/p><\/div><div class=\"txlede-b\"><h3>From film to blocked gland<\/h3><p>The biofilm matters because of what it does once it is thick enough. Bacterial density inside it eventually crosses a threshold at which the bacteria switch on genes they keep quiet at lower numbers, and they begin releasing toxins and lipases.<\/p><p>Those lipases break meibum down into free fatty acids, which are inflammatory in their own right and which raise the melting point of the oil that remains, so it is stiffer and less able to leave the gland.<\/p><p>Alongside this, the biofilm and any Demodex mites present physically plug and keratinise the gland openings along the margin. Removing the film addresses a cause of gland blockage rather than only its consequences.<\/p><\/div><\/div><\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">03<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"the-instruments\">The instruments<\/h2>\n<p class=\"txdevnote\">Smart Dry Eyes is not tied to any manufacturer. Most people describe a tickle rather than discomfort. Five to ten minutes.<\/p><\/li><li><h3>After<\/h3><p>You leave immediately. The lid margin may look slightly pink for an hour and can sting briefly. There are no restrictions.<\/p><\/li><li><h3>Keeping it up<\/h3><p>In-practice cleaning is typically repeated every four to six months. What happens between those visits is daily lid hygiene at home, which is where most of the long-term result comes from.<\/p><\/li><\/ol>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txsplit\"><div><div class=\"txhead\"><span class=\"txnum\">05<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"what-the-evidence-shows-for-eyelid-cleansing\">What the evidence shows for eyelid cleansing<\/h2><\/div><div><div class=\"txev\"><div class=\"txevr\"><p class=\"k\">Randomised trial, 2024, 40 patients<\/p><div class=\"v\"><p>We are going to be straightforward about this, because the marketing in this area is not. In 2024 a German university team ran a double-masked, randomised trial of a single powered blepharoexfoliation session against a sham procedure in 40 patients with blepharitis. The sham group did numerically better on the symptom score, and was the only group to improve significantly on a clinical grading scale. The authors concluded that the treatment could not be recommended for blepharitis on that evidence. The trial had no industry funding and no declared conflicts. It tested one session with four weeks of follow-up, and it did not measure bacterial load or gland function, which are limitations, but it is the best-designed study that exists.<\/p><\/div><\/div><div class=\"txevr\"><p class=\"k\">The 60 to 66 per cent claim<\/p><div class=\"v\"><p>The claim, repeated on many clinic sites, that powered lid cleaning increases tear break-up time by 60 to 66 per cent traces back to small uncontrolled case series and is contradicted by the trial above. We do not use it. The biofilm theory that underpins most of the marketing was published by the founder and chief executive of the company that makes the best-known device; it is a reasonable mechanism, not evidence of effect.<\/p><\/div><\/div><div class=\"txevr txevr--pos\"><p class=\"k\">Debridement-scaling, and our position<\/p><div class=\"v\"><p>What does have support is manual debridement-scaling. In a controlled study of 28 patients, scaling the keratinised lid margin improved symptom scores and increased the number of functioning meibomian glands, while the untreated group did not change. Small and unmasked, but positive and independent.
