It is also the part of the programme most often given alongside something else, because it works on a different mechanism from heat and from pulsed light.<\/p><p>It is not a substitute for warming or expression. Under chronic inflammation, nitric oxide binds to that enzyme and slows electron flow, which is a little like a partially blocked exhaust.<\/p><p>The light is thought to displace the nitric oxide, electron flow resumes, and the cell produces more ATP. A brief, controlled rise in reactive oxygen species follows, and that acts as a signal rather than as damage, shifting the cell&#8217;s inflammatory gene expression. In the eyelid the proposed result is better-functioning secretory cells and a quieter local environment.<\/p><\/div><div class=\"txlede-b\"><h3>Blue, at 465 nanometres<\/h3><p>Blue light at around 465 nanometres does something different and simpler. It excites porphyrins inside the bacteria that live on the lid margin and around Demodex mites, producing singlet oxygen that damages them from within. That is why blue is used for blepharitis and Demodex, and red for gland function.<\/p><\/div><div class=\"txlede-b\"><h3>Where the account stops<\/h3><p>Two honest limits. The cytochrome c oxidase account is well supported in cell and animal studies but has never been demonstrated directly in a living human meibomian gland. And the dose-response is biphasic: there is a window, and more light is not better.<\/p><\/div><\/div><div class=\"txcall\"><p>Low-level light therapy does not heat the meibomian glands. The energy involved is roughly a hundred times too small to raise the inner lid to the temperature that melts hardened meibum. Pages that describe it as melting the oil are describing a different treatment.<\/p><\/div><\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">03<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"the-devices\">The devices<\/h2>\n<p class=\"txdevnote\">Smart Dry Eyes is not tied to any manufacturer. The systems below are the ones most often used for this treatment in our partner centres and in European practice generally. No anaesthetic, no gel, no shields beyond keeping the eyes closed.<\/p><\/li><li><h3>During<\/h3><p>The mask or panel sits over the closed eyes for ten to seventeen minutes. Most patients describe it as pleasant. Many centres run it directly after pulsed light, so the two share one appointment.<\/p><\/li><li><h3>After<\/h3><p>You leave and carry on. There is no restriction of any kind, including driving and screens.<\/p><\/li><li><h3>The course<\/h3><p>Protocols vary more than in other treatments. Four sessions is typical, given either every other day across one week or spread over twelve weeks alongside pulsed light.<\/p><\/li><\/ol>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txsplit\"><div><div class=\"txhead\"><span class=\"txnum\">05<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"what-the-evidence-shows-for-low-level-light-\">What the evidence shows for low-level light therapy<\/h2><\/div><div><div class=\"txev\"><div class=\"txevr\"><p class=\"k\">Placebo-controlled, publicly funded, 2022<\/p><div class=\"v\"><p>The trial worth knowing is a randomised, observer-masked study with a genuine placebo arm, funded by a government health institute rather than by a manufacturer (Park and colleagues, Scientific Reports, 2022). Forty patients were split evenly. Corneal staining improved by 1.10 points against 0.40 in the placebo group, and the Schirmer test rose by 2.18 millimetres against a fall of 0.63. It is a small study, but it is the cleanest one in this field and it supports the modality.<\/p><\/div><\/div><div class=\"txevr\"><p class=\"k\">Where it ranks among devices<\/p><div class=\"v\"><p>A network meta-analysis covering 45 randomised trials and 3,455 patients ranked pulsed light combined with low-level light third of all device treatments for tear break-up time. That ranking is for the combination, so it cannot tell us how much of the effect the light mask contributed on its own.<\/p><\/div><\/div><div class=\"txevr txevr--pos\"><p class=\"k\">Why we discount the device studies<\/p><div class=\"v\"><p>The studies of specific commercial masks are considerably weaker: retrospective, uncontrolled, with as few as seventeen patients, and frequently written by paid consultants of the manufacturer. In one of them the patients were also taking oral doxycycline, which treats meibomian gland disease perfectly well by itself. In Germany these are private-pay treatments: statutory insurers do not reimburse them, private insurers and Beihilfe often do, in part. Your centre confirms the figure before anything begins.<\/p>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb txb--tint\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">08<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"light-therapy-questions\">Light therapy questions<\/h2>\n<div class=\"txfaq\"><details><summary>Is it the same as the red light panels sold for skin?<\/summary><div class=\"a\"><p>The physics is the same and some practices use exactly those panels. The difference is dose and control: an eye protocol specifies wavelength, output and time, and is given by someone who has looked at your lid margins first. Note also that most of these panels are cleared for skin and pain, not for the eye, so their use here is off-label even where it is reasonable.<\/p><\/div><\/details><details><summary>Do I need to close my eyes?<\/summary><div class=\"a\"><p>Yes, and that is all the protection needed. The light is low-powered and non-coherent, unlike a laser, but the eyelid is the intended target and there is no reason to expose the surface of the eye.<\/p><\/div><\/details><details><summary>Why is it usually combined with something else?<\/summary><div class=\"a\"><p>Because it does one job. Light therapy calms inflammation and supports the secretory cells. It does not soften hardened oil and it does not empty a blocked duct.
