The clearest sign is visible: fine dilated vessels running along the lid margin, often together with facial rosacea or a permanently flushed look across the cheeks and nose. Ocular rosacea and IPL belong together, and it is the treatment of choice when the lid margin looks angry rather than merely clogged.<\/p><p>It is the wrong choice for dry eye caused by too few tears rather than by evaporation. If the glands themselves are gone on meibography, there is no gland function left for IPL to restore.<\/p><\/div><\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb txb--tint\">\n<div class=\"txw\">\n<div class=\"txsplit\"><div><div class=\"txhead\"><span class=\"txnum\">02<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"how-ipl-works-on-the-lid-margin\">How IPL works on the lid margin<\/h2><figure class=\"txchain\"><figcaption>Mechanism<\/figcaption><ol><li><span class=\"n\">01<\/span><span class=\"t\">Broadband light on cheek, nose and temple<\/span><\/li><li><span class=\"n\">02<\/span><span class=\"t\">Haemoglobin absorbs it and heats<\/span><\/li><li><span class=\"n\">03<\/span><span class=\"t\">The abnormal vessel closes<\/span><\/li><li><span class=\"n\">04<\/span><span class=\"t\">Inflammatory mediators stop arriving<\/span><\/li><li><span class=\"n\">05<\/span><span class=\"t\">The gland bed settles<\/span><\/li><\/ol><\/figure><\/div><div><div class=\"txprose txprose--blocks\"><div class=\"txlede-b\"><h3>Where the light goes<\/h3><p>The handpiece emits a broad band of light rather than a single laser wavelength, and it is placed on the cheek, the bridge of the nose and the temple, never over the open eye.<\/p><p>Haemoglobin in the small abnormal vessels absorbs that light more strongly than the surrounding skin does, heats, and the vessel closes. This is the same principle used to clear thread veins in dermatology.<\/p><\/div><div class=\"txlede-b\"><h3>What travels in them<\/h3><p>Those vessels matter because of what travels in them. In chronic lid inflammation they carry inflammatory mediators directly to the gland bed, and studies that sampled tears before and after IPL have measured falls in interleukin-6, interleukin-17A, tumour necrosis factor alpha and prostaglandin E2.<\/p><p>Close the supply route and the glands sit in a quieter environment. Two secondary effects are also documented: the light warms the meibum a little, and it appears to reduce Demodex mites living in the lash follicles.<\/p><\/div><div class=\"txlede-b\"><h3>The parasympathetic hypothesis<\/h3><p>One mechanism you will see quoted deserves a caution. The manufacturer of the E>Eye device proposes that its regulated pulse train stimulates the parasympathetic nerves supplying the glands. That is the company&#8217;s hypothesis. It is not established, and the independent reviews of how pulsed light works in dry eye do not describe it.<\/p><\/div><\/div><div class=\"txcall\"><p>Protective shields are placed over the eyes for every flash, without exception. Intense broad-spectrum light reaching the pigmented tissue inside the eye can cause lasting damage to the iris.<\/p><\/div><\/div><\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">03<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"the-devices\">The devices<\/h2>\n<p class=\"txdevnote\">Smart Dry Eyes is not tied to any manufacturer. The systems below are the ones most often used for this treatment in our partner centres and in European practice generally. Smart Dry Eyes has no commercial relationship with these manufacturers.<\/p>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb txb--tint\">\n<div class=\"txw\">\n<div class=\"txhead\"><span class=\"txnum\">04<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"what-an-ipl-session-is-like\">What an IPL session is like<\/h2>\n<ol class=\"txsteps\"><li><h3>Before<\/h3><p>Your skin type is graded on the Fitzpatrick scale and the energy is set from it. Opaque shields go over the eyes and a cooling gel is applied to the cheeks and temples.<\/p><\/li><li><h3>During<\/h3><p>Four or five flashes along the lower periocular skin from the nose outwards, and one at the temple to reach the upper lids. Each feels like a warm snap. Five to ten minutes in total.<\/p><\/li><li><h3>After<\/h3><p>The gel is wiped off and you can put make-up straight back on. Mild pinkness in the treated skin sometimes lasts an hour or two.<\/p><\/li><li><h3>The course<\/h3><p>Three sessions on days 1, 15 and 45, with a fourth around day 75 for severe cases. Manufacturers report benefit lasting at least six months, with a maintenance session about once a year.<\/p><\/li><\/ol>\n<\/div>\n<\/div>\n\n\n\n<div class=\"txb\">\n<div class=\"txw\">\n<div class=\"txsplit\"><div><div class=\"txhead\"><span class=\"txnum\">05<\/span><span class=\"txrule\"><\/span><\/div><h2 class=\"txh\" id=\"what-the-evidence-shows-for-ipl\">What the evidence shows for IPL<\/h2><\/div><div><div class=\"txev\"><div class=\"txevr\"><p class=\"k\">Sham-controlled and masked, 2015<\/p><div class=\"v\"><p>The best-designed single study treated one eye and gave the other a sham, with both patient and assessor masked (Craig and colleagues, Investigative Ophthalmology &#038; Visual Science, 2015). Lipid layer grade improved in 82 per cent of treated eyes, non-invasive break-up time went from 5.3 to 14.1 seconds, and symptoms improved. Two measures did not move at all: the rate at which tears evaporated, and tear osmolarity.<\/p><\/div><\/div><div class=\"txevr\"><p class=\"k\">Cochrane 2020, and what came after<\/p><div class=\"v\"><p>A Cochrane review in 2020 pooled the randomised trials available at the time and concluded that whether IPL modifies the symptoms or signs of evaporative dry eye was uncertain, on low to very low certainty evidence. That finding is five years old and rests on three small trials.
